Come join our Dynamic Team of Caring Professionals! RN CASE MANAGER QUALIFICATIONS NEEDED:
Current OK State RN Licensure, 1-2 years Home Health Experience Preferred Oasis E Training Good Organizational, Time Management and Communications Skills RN CASE MANAGER
Responsibilities:
Completes Initial Assessments (SOC) to determine home care needs Develops a Plan of Care which establishes goals based on nursing diagnosis and incorporates therapeutic, preventative, and rehabilitative nursing procedures.
Regularly evaluate plans of care and Performs Recertifications every 60 days.
Educates patients and their families on Plan of Care, Disease Management, Medication Management; all other aspects of Care.
Coordinates Patient Care with Physicians offices, other referral sources, community resources.
Participates in On-Call Duties once a moth as defined by On-Call Policy.
