Job Description:
To provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual Consumer needs.
The overall goal of the position is to enhance the quality of client management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management and other community resources.
This position is accountable to the Supervisor of Case Management.
QUALIFICATIONS Bachelor's degree in a related field is required.
SUMMARY OF MAJOR AND ESSENTIAL FUNCTIONS Provides case management so that client progresses through the continuum of care and is discharged to the least restrictive environment.
Coordinate the integration of the social service function into client care.
Adhere to departmental goals, objectives, standards of performance, and policies and procedures.
PRIMARY
Responsibilities:
Conduct field visits to perform Face to Face Assessments with Consumers.
Development and maintain social relationships Exploring interests, choices and capacities Identifying meaningful and productive goals and activities Making safe and growth-oriented decisions Developing self advocacy skills Acts as an advocate for an individual's needs.
Monitor service plans to assure quality and the satisfaction of the individual and family.
Connect families and individuals to community organizations, resource and service providers.
Collect information to help families and individuals make informed decisions.
Help represent the concerns of families and individuals to those providing services to them.
Help foster independence through the use of self-advocacy skills.
Consumers receive appropriate, quality services; Services are modified to meet changing Consumer needs; Interventions are made to ensure the system is responsive to Consumers; and The plan of care is progressively moving the individual toward self-care and preventing further loss of function; Establishes short and long term goals in collaboration with the member that meet the member's needs and the referral source's requirements.
Communicate care objectives to appropriate individuals/departments/referral sources.
Identify members with potential for high risk complications and coordinate appropriate treatment in conjunction with the member and the health care team.
Establishes working relationships with referral sources and community resources.
Evaluate quality of necessary medical services.
Document effectiveness of service coordination services.
Collaboratively works with other clinical team members and other departments.
Participates in Quality Improvement processes.
Serve on internal and external committees.
Be the Service Coordinator and Consumer Consultant.
The case manager facilitates interdisciplinary team processes to:
identify Consumer needs and strengths; identify and coordinate service delivery; arrange for and initiate service implementation, and; monitor service delivery; Assists Consumers to recognize and identify their service needs; Educates Consumers on the quality and appropriateness of services; Supports Consumers to take responsibility for their own care to the greatest extent possible; and, when necessary, represents Consumers in interactions with DHS program administration; and Be the Gatekeeper.
The case manager assures level of care and program appropriateness:
Only those Consumers appropriate for Medicaid case management receive the service through the DHS programs; To the extent possible, methods are built into the service plan to progressively move the Consumer toward independence and the use of informal, family and volunteer services; Expenditure of funds is justified; and Costs are monitored and contained.
Coordinates the provision of social services to patients, families, and significant others to enable them to deal with the impact of illness on individual family functioning and to achieve maximum benefits from healthcare services.
Maintain strict member confidentiality and comply with all HIPAA requirements.
